Summary
This form may be attached to your Preliminary Change in Ownership Report (PCOR) and included with your document to be recorded if it is required to show the transfer tax amount when securing or terminating financing. The Documentary Transfer Tax (DTT) imposes a tax on each deed, instrument, or writing by which any lands, tenements, or any other realty sold, shall be granted, transferred, or otherwise conveyed to another person.
This form must have original signatures before it is submitted.
There are two ways to submit the form:
