Documentary Transfer Tax Transmittal

This form may be attached to your Preliminary Change in Ownership Report (PCOR) and included with your document to be recorded if it is required to show the transfer tax amount. The Documentary Transfer Tax (DTT) imposes a tax on each deed, instrument, or writing by which any lands, tenements, or any other realty sold, shall be granted, transferred, or otherwise conveyed to another person.

This form must have original signatures before it is submitted.

There are two ways to submit the form:

2. Print and complete by hand: Print a blank PDF version of the form, fill it out manually, and submit it in person or by mail.
